Tools to assess if a report is required

We provide a number of tools to help suppliers assess if a mandatory report is required.

Example notice for a supplier's employees about mandatory reporting obligations

You are responsible for reporting regardless of who in your organisation becomes aware of a reportable incident.

Employees, contractors, and any person representing or acting as an agent of your business need to be aware of these requirements. You should regularly remind them about the mandatory reporting obligations.

This notice is an example of information that should be provided to employees to ensure effective complaint handling and mandatory reporting processes.

What you need to do

If you become aware of any incident where our product or service may have caused a death or serious injury or illness, you must report it to [insert supplier contact point] immediately.

This includes (but is not limited to):

  • Notifications through customer complaints,
  • Information provided by customers in writing or verbally,
  • News or media reports, or
