  • You are responsible for reporting regardless of who in your organisation becomes aware of a reportable incident.
  • You should routinely remind your employees, contractors, and other representatives about the mandatory reporting obligations.

This notice is an example of information that should be provided to employees to ensure effective complaint handling and mandatory reporting processes.

Employees, contractors, and any other persons representing or acting as an agent of your business need to be aware of these requirements.

What you need to do

If you become aware of any incident where our product or service may have caused a death or serious injury or illness, you must report it to [insert supplier contact point] immediately.

A ‘serious’ injury or illness is one where:

  1. the physical injury or illness is acute
  2. the physical injury or illness requires medical or surgical treatment
  3. treatment by, or under the supervision of, a medical practitioner or a nurse was required (whether or not in a hospital, clinic, or similar place).

It doesn’t matter how you become aware of the death or serious injury or illness; you must report it to [insert supplier contact point].

This includes (but is not limited to):

  • Notifications through customer complaints,
  • Information provided by customers in writing or verbally,
  • News or media reports, or
  • Comments made on social media.
