Eaton Industries P/L—Holec Dumeco DMV250 & DMV400 switch disconnectors

Identifying features

Standard distribution boards, modular distribution boards & custom LV switchboards fitted with Quicklag or DIN MCB chassis AND Holec Dumeco main switches of 250A or 400A rating, manufactured by Cutler-Hammer or Eaton. Specifically excluded are similar switchboards manufactured by Email-Westinghouse or Email Switchgear.

What are the defects?

The handle may fail to operate as intended under normal use due to stress cracks that occur in the molded socket that connects the handle to the on/off switching shaft.

What are the hazards?

In the event that the handle socket cracks, there is no immediate hazard.
Subsequent events may introduce an electric shock hazard IF:
1. user opens the dead-front escutcheon AND
2. user does not check that the contacts of the switch disconnector are visually open AND
3. user does not conduct appropriate voltage measurement to confirm that the load has been isolated from the supply.
