What are the defects?

Product is potentially dangerous if used without restraining the infant with the safety harness. Infants may roll from their backs to their stomach or side if unrestrained.

What are the hazards?

There is a risk of suffocation if infants roll from their back to their stomach or side while unrestrained, which can result in severe injury or death. Infant fatalities have occurred overseas in Rock ‘n Play Sleepers, after the infants rolled from their back to their stomach or side while unrestrained.

What should consumers do?

Consumers should immediately cease using the product. Consumers should contact Mattel by email at infoaust@mattel.com or by phone on 1300 135 312 (6:30 a.m. – 3:00 p.m. Melbourne time Mon – Fri). After contacting Mattel, consumers will be provided with instructions on how to remove the "hubs" of the product, which are attached to the legs of the product, and will be directed to send the hubs back to Mattel in the postage-paid envelope that will be provided.
