250A & 400A switch disconnectors fitted to Cutler-Hammer and Eaton Low Voltage Switchboards manufactured from May 2001 to 16th March 2009. Product supplied after 16th March 2009 should be checked to determine if it contains a switch disconnector from the effected period.

Reason the product is recalled

The handle may fail to operate as intended under normal use due to stress cracks that occur in the molded socket that connects the handle to the on/off switching shaft.

The hazards to consumers

In the event that the handle socket cracks, there is no immediate hazard.
Subsequent events may introduce an electric shock hazard IF:
1. user opens the dead-front escutcheon AND
2. user does not check that the contacts of the switch disconnector are visually open AND
3. user does not conduct appropriate voltage measurement to confirm that the load has been isolated from the supply.
